- For all spine-related referrals we partner with the Salem Health Spine Center Spine Center | Salem Health where a group of triage nurses accelerates treatment for neck, spine, and back pain. To make a referral, call the Salem Health Spine Center at 503-814-2225 or fax a referral form, along with records that pertain to the spine problem, to the Salem Health Spine Center at 503-814-5495.
